New Student Enrollment

Welcome to DVHS, home of the Panthers! We look forward to working with you to help your child succeed in high school.

To enroll your child at DVHS, please bring the following items at the time of registration:

  • Birth Certificate (Certified Copy)
  • Immunization Records
  • Transfer Grades (if coming from another school during the school year)
  • Unofficial transcript

Please complete and submit the following items (found in the Enrollment Packet that can be downloaded by clicking here or picked up at the HS office):

  • Records Request
  • High School Enrollment Form
  • Special Services Background Information
  • South Dakota Immunization System (SDIIS) Access Agreement
  • Acceptable Use of Internet/Technology Form
  • Mobile Computer Protection Form
  • Over the Counter Medication Administration Form
  • Prescription Medication Form (if applicable)
  • Computer Insurance Form
Online Restricted Drivers License Renewal Option

Transcript Request

To request an official high school transcript, please e-mail:

angela.bernard@k12.sd.us or fax a request to: 605-422-3827.
Please include "Request Transcript" in the subject line of your email or your email maybe be rejected.
Please include:
  • Your Full Name (at the time of graduation)
  • Year of Graduation
  • Date of Birth
  • Complete Address of Where to Send Your transcript
Please allow 48 hours for your request to be completed.

This is a reminder for curfew times as agreed upon for the Tri State Area by the city ordinances ~ Siouxland Cares

TRI-STATE CURFEW TIMES
If you’re serious about preventing alcohol and other drug abuse in Siouxland…Know the curfew laws and enforce them in your home.
 
Minors under the age of 14 - 10:00pm
Minors ages 14 and 15 - 11:00pm
Minors ages 16 and 17 - 12:00am (Midnight)
